🩺 Sample Clinical Scenarios

👤 Patient Information

📐 A. Extent (Body Surface Area %)

Estimate the percentage of each body region affected by eczema (0-100%)

🔥 B. Intensity (Score 0-3 each)

Assess each item on a representative affected area: 0=absent, 1=mild, 2=moderate, 3=severe

😓 C. Subjective Symptoms (VAS 0-10)

Patient-reported symptoms over the last 3 days/nights

Step-by-Step Calculation

SCORAD CALCULATION

SCORAD = A/5 + 7B/2 + C, where A=extent, B=intensity, C=subjective

COMPONENT A: EXTENT (Body Surface Area)

Extent calculation (adult proportions)

A = 0.0% BSA affected

COMPONENT B: INTENSITY (6 items, 0-3 each)

B = 0 + 0 + 0 + 0 + 0 + 0 = 0/18

COMPONENT C: SUBJECTIVE SYMPTOMS (VAS 0-10)

C = Pruritus (0) + Sleep Loss (0) = 0/20

TOTAL SCORAD SCORE

SCORAD = A/5 + 7B/2 + C

SCORAD = 0.0/5 + 7×0/2 + 0

SCORAD = 0.00 + 0.00 + 0 = 0.0

Objective SCORAD (A/5 + 7B/2) = 0.0

INTERPRETATION

SCORAD 0.0 = Mild (Less than 25)

SCORAD Severity Classification

Score RangeSeverityClinical Significance
Less than 25MildLimited disease, good response to basic therapy
25-50ModerateSignificant disease impact, requires active management
Greater than 50SevereSignificant quality of life impact, specialist management needed

🩺 What is SCORAD?

The SCORing Atopic Dermatitis (SCORAD) index was developed by the European Task Force on Atopic Dermatitis in 1993 as a standardized tool to assess the severity of atopic dermatitis (eczema).

It combines objective physician assessment (extent and intensity) with patient-reported symptoms (pruritus and sleep disturbance) for a comprehensive severity score.

Score Interpretation

  • Less than 25: Mild atopic dermatitis
  • 25-50: Moderate atopic dermatitis
  • Greater than 50: Severe atopic dermatitis

📋 How to Calculate SCORAD

  1. Assess Extent (A): Estimate % of each body region affected (Rule of 9s)
  2. Assess Intensity (B): Score 6 items (0-3 each) on a representative area
  3. Record Symptoms (C): Patient rates pruritus and sleep loss (VAS 0-10)
  4. Calculate: SCORAD = A/5 + 7B/2 + C

Formula: SCORAD = A/5 + 7B/2 + C

Maximum possible score: ~103

When to Use SCORAD

Clinical Applications

  • • Initial severity assessment
  • • Treatment response monitoring
  • • Clinical trial endpoints
  • • Specialist referral decisions
  • • Systemic therapy eligibility

Assessment Frequency

  • • At baseline for new patients
  • • Every 2-4 weeks during flares
  • • Every 1-3 months on maintenance
  • • Before and after treatment changes

📐 SCORAD Formula

SCORAD = A/5 + 7B/2 + C

A = Extent (0-100% BSA)

B = Intensity (0-18, sum of 6 items)

C = Subjective (0-20, pruritus + sleep loss)

Objective SCORAD = A/5 + 7B/2 (without subjective symptoms)
