Clinical Recommendations
Step-by-Step Calculation
The ICH Score predicts 30-day mortality in intracerebral hemorrhage patients using 5 clinical and imaging factors.
GCS: 15 (Mild/Normal (13-15))
Points: 0 (3-4 = 2pts, 5-12 = 1pt, 13-15 = 0pts)
Volume: 20 mL
Points: 0 (≥30 mL = 1pt, <30 mL = 0pts)
IVH Present: No
Points: 0 (Yes = 1pt, No = 0pts)
Infratentorial Origin: No (supratentorial)
Points: 0 (Infratentorial = 1pt, Supratentorial = 0pts)
Age: 60 years
Points: 0 (≥80 years = 1pt, <80 years = 0pts)
ICH Score = GCS + Volume + IVH + Location + Age
ICH Score = 0 + 0 + 0 + 0 + 0 = 0
30-Day Mortality: 0%
Prognostic Category: Excellent prognosis
ICH Score Mortality Reference

| 2 | 26% | Moderate mortality risk |
| 3 | 72% | High mortality risk |

📋 What is the ICH Score?
The ICH Score was developed by Hemphill et al. in 2001 as a simple, validated clinical grading scale to predict 30-day mortality in patients with spontaneous intracerebral hemorrhage (ICH). ICH is the deadliest form of stroke, with mortality rates of 40-50%.
Five Scoring Components
- • GCS: 3-4 (2pts), 5-12 (1pt), 13-15 (0pts)
- • ICH Volume: ≥30 mL (1pt), less than 30 mL (0pts)
- • Intraventricular hemorrhage: Yes (1pt), No (0pts)
- • Infratentorial origin: Yes (1pt), No (0pts)
- • Age: ≥80 years (1pt), less than 80 years (0pts)
🔧 How to Calculate ICH Volume
ABC/2 Method
- A = Largest diameter (cm)
- B = Perpendicular diameter (cm)
- C = Number of slices × slice thickness (cm)
