Step-by-Step Calculation

ICH SCORE CALCULATION

The ICH Score predicts 30-day mortality in intracerebral hemorrhage patients using 5 clinical and imaging factors.

COMPONENT SCORING
Glasgow Coma Scale (GCS)

GCS: 15 (Mild/Normal (13-15))

Points: 0 (3-4 = 2pts, 5-12 = 1pt, 13-15 = 0pts)

ICH Volume

Volume: 20 mL

Points: 0 (≥30 mL = 1pt, <30 mL = 0pts)

Intraventricular Hemorrhage (IVH)

IVH Present: No

Points: 0 (Yes = 1pt, No = 0pts)

ICH Location

Infratentorial Origin: No (supratentorial)

Points: 0 (Infratentorial = 1pt, Supratentorial = 0pts)

Age

Age: 60 years

Points: 0 (≥80 years = 1pt, <80 years = 0pts)

TOTAL ICH SCORE

ICH Score = GCS + Volume + IVH + Location + Age

ICH Score = 0 + 0 + 0 + 0 + 0 = 0

📋 What is the ICH Score?

The ICH Score was developed by Hemphill et al. in 2001 as a simple, validated clinical grading scale to predict 30-day mortality in patients with spontaneous intracerebral hemorrhage (ICH). ICH is the deadliest form of stroke, with mortality rates of 40-50%.

Five Scoring Components

  • • GCS: 3-4 (2pts), 5-12 (1pt), 13-15 (0pts)
  • • ICH Volume: ≥30 mL (1pt), less than 30 mL (0pts)
  • • Intraventricular hemorrhage: Yes (1pt), No (0pts)
  • • Infratentorial origin: Yes (1pt), No (0pts)
  • • Age: ≥80 years (1pt), less than 80 years (0pts)

🔧 How to Calculate ICH Volume

ABC/2 Method

Volume (mL) = (A × B × C) / 2
  • A = Largest diameter (cm)
  • B = Perpendicular diameter (cm)
  • C = Number of slices × slice thickness (cm)
