The Revised Trauma Score (RTS) is a physiologic scoring system developed by Champion et al. in 1989 to predict survival in trauma patients. It uses three easily obtainable vital signs weighted according to their predictive value.

The RTS is used in two forms: the Triage RTS (simple sum of coded values, 0-12) for field triage decisions, and the weighted RTS (0-7.84) for outcome prediction and quality assurance.
