Step-by-Step Calculation
The RTS uses weighted physiologic parameters to predict trauma survival.
GCS 15 โ Coded Value = 4
SBP 120 mmHg โ Coded Value = 4
RR 16/min โ Coded Value = 4
Triage RTS = 4 + 4 + 4 = 12
RTS = 0.9368 ร GCS(c) + 0.7326 ร SBP(c) + 0.2908 ร RR(c)
RTS = (0.9368 ร 4) + (0.7326 ร 4) + (0.2908 ร 4)
RTS = 3.7472 + 2.9304 + 1.1632 = 7.8408
RTS 7.84 = Mild
Estimated Survival Probability: 98.5%
RTS Reference
| RTS Range | Severity | Survival Estimate | Action |
|---|---|---|---|
| 0-3.99 | Critical | Less than 25% | Immediate trauma center transfer |
| 4-5.99 | Severe | 25-50% | Trauma center transport |
| 6-6.99 | Moderate | 50-75% | Consider trauma center |
| 7-7.84 | Mild | Greater than 75% | Standard trauma evaluation |

๐ What is the Revised Trauma Score?
The Revised Trauma Score (RTS) is a physiologic scoring system developed by Champion et al. in 1989 to predict survival in trauma patients. It uses three easily obtainable vital signs weighted according to their predictive value.
The RTS is used in two forms: the Triage RTS (simple sum of coded values, 0-12) for field triage decisions, and the weighted RTS (0-7.84) for outcome prediction and quality assurance.
RTS Components
- โข Glasgow Coma Scale (GCS): Weight 0.9368
- โข Systolic Blood Pressure: Weight 0.7326
- โข Respiratory Rate: Weight 0.2908
๐ How to Calculate RTS
- Assess GCS: Score 3-15 (convert to coded value 0-4)
- Measure SBP: In mmHg (convert to coded value 0-4)
- Count RR: Breaths per minute (convert to coded value 0-4)
- Calculate Triage RTS: Sum of three coded values (0-12)
- Calculate Weighted RTS: Apply coefficients to coded values
RTS = 0.9368 ร GCS(c) + 0.7326 ร SBP(c) + 0.2908 ร RR(c)
Maximum score: 7.8408 (all coded values = 4)
โฐ When to Use RTS
Clinical Applications
- โข Pre-hospital triage decisions
- โข Trauma center referral criteria
- โข Survival probability estimation
- โข Trauma registry data
- โข Quality improvement programs
Triage Criteria
- โข T-RTS less than 11: Transport to trauma center
- โข RTS less than 4: Consider futility assessment
- โข RTS 4-6: High-risk, needs aggressive care
- โข RTS greater than 7: Generally favorable prognosis
