Patient meets discharge criteria - proceed with discharge planning

Step-by-Step Assessment

Aldrete Score Components

Activity Score: 2/2 - Moves all extremities

Respiration Score: 2/2 - Breathes deeply, coughs freely

Circulation Score: 2/2 - BP ยฑ20% of preanesthetic

Consciousness Score: 2/2 - Fully awake

Oxygen Saturation Score: 2/2 - SpO2 >92% on room air

Total Score Calculation

Total = 2 + 2 + 2 + 2 + 2 = 10/10

Clinical Interpretation

Category: Ready for Discharge (9-10)

Patient meets discharge criteria with excellent recovery

Recommendation: May proceed to Phase II recovery or discharge with escort

What is the Aldrete Score?

The Aldrete Score is a medical scoring system developed by Dr. Jorge Antonio Aldrete in 1970 to assess patient recovery from anesthesia. It evaluates five key physiological parameters to determine when a patient is ready for discharge from the Post-Anesthesia Care Unit (PACU) to a general ward or home.

๐Ÿฅ

Standardized Assessment

Provides objective criteria for evaluating post-anesthesia recovery, reducing subjective variability in discharge decisions.

โœ…

Patient Safety

Ensures patients meet minimum recovery criteria before transfer, reducing post-operative complications.

๐Ÿ“Š

Five Key Criteria

Evaluates Activity, Respiration, Circulation, Consciousness, and Oxygen Saturation - each scored 0-2.

How Does the Aldrete Score Work?

Scoring Methodology

Assessment Process

  1. 1Evaluate motor activity (ability to move extremities)
  2. 2Assess respiratory function and airway patency
  3. 3Compare blood pressure to preanesthetic baseline
  4. 4Evaluate level of consciousness
  5. 5Check oxygen saturation levels

Score Interpretation

9-10:Ready for discharge
7-8:Nearly ready, continue monitoring
5-6:Requires continued PACU care
0-4:Critical - intensive monitoring needed

When to Use the Aldrete Score

Post-Operative Assessment

  • โ€ข After general anesthesia procedures
  • โ€ข Following regional anesthesia (spinal/epidural)
  • โ€ข After sedation/MAC procedures
  • โ€ข Ambulatory surgery discharge decisions

Monitoring Frequency

  • โ€ข Every 15 minutes initially in PACU
  • โ€ข Before transfer to ward
  • โ€ข Before ambulatory discharge
  • โ€ข When clinical status changes

Aldrete Score Calculation

Total Score Formula

Aldrete Score = Activity + Respiration + Circulation + Consciousness + SpO2

Each component scored 0-2, Total range: 0-10

Discharge Threshold

Discharge Ready = Total Score >= 9
