What are RBC Indices?

RBC Indices are calculated values that describe the size and hemoglobin content of red blood cells. They are essential for classifying anemias and guiding diagnostic workup.

MCV (Mean Corpuscular Volume)

Average RBC size in femtoliters (fL). Normal: 80-100 fL

MCH (Mean Corpuscular Hemoglobin)

Average Hgb per RBC in picograms (pg). Normal: 27-33 pg

MCHC (Mean Corpuscular Hgb Concentration)

Average Hgb concentration in g/dL. Normal: 32-36 g/dL

How Are RBC Indices Used?

RBC indices help classify anemias morphologically, which guides the differential diagnosis and appropriate workup. The combination of MCV and MCHC creates distinct patterns for different types of anemia.

Anemia Classification by MCV:

Microcytic (MCV < 80): Iron deficiency, Thalassemia, Lead poisoning, Sideroblastic
Normocytic (MCV 80-100): Chronic disease, CKD, Hemolysis, Acute blood loss
Macrocytic (MCV > 100): B12/Folate deficiency, MDS, Liver disease, Hypothyroidism

RBC Indices Formulas

MCV Formula:

MCV (fL) = (Hematocrit % / RBC count in M/ฮผL) ร— 10

MCH Formula:

MCH (pg) = (Hemoglobin g/dL / RBC count in M/ฮผL) ร— 10

MCHC Formula:

MCHC (g/dL) = (Hemoglobin g/dL / Hematocrit %) ร— 100
