What is Mean Sac Diameter (MSD)?
Mean Sac Diameter (MSD) is the average of three perpendicular measurements of the gestational sac seen on early pregnancy ultrasound. It is one of the earliest ways to date a pregnancy, typically used between 4-8 weeks when the embryo may not yet be visible. The gestational sac is the first structure visible on ultrasound, appearing as a small fluid-filled circle within the uterus.
Earliest Dating Method
MSD can date pregnancies as early as 4-5 weeks, before the embryo is visible.
Viability Assessment
Helps assess if expected structures (yolk sac, embryo, heartbeat) are visible at the right time.
Growth Tracking
MSD grows approximately 1mm per day, allowing monitoring of early pregnancy growth.
How is MSD Measured and Used?
Measurement Technique
How to Measure
- โข Measure inner-to-inner borders of the sac
- โข Take 3 perpendicular measurements
- โข Calculate average of all three
- โข Exclude yolk sac from measurement
Expected Findings by MSD
- โข MSD 8mm+: Yolk sac should be visible
- โข MSD 16mm+: Embryo may be visible
- โข MSD 25mm+: Embryo must be visible
When is MSD Dating Used?
Very Early Pregnancy
Dating before embryo is visible (4-6 weeks)
Pregnancy of Unknown Location
Confirming intrauterine pregnancy
Follow-up Scans
Monitoring early pregnancy growth
Viability Assessment
Checking if structures appear on schedule
IVF Pregnancies
Early confirmation of implantation
Before CRL Available
When embryo too small to measure

Frequently Asked Questions
What if there's no yolk sac visible?
A yolk sac should be visible when MSD reaches 8mm. If the MSD is smaller, it may be too early. If MSD is larger than 8mm without a yolk sac, a follow-up ultrasound in 7-10 days is typically recommended.
What is the discriminatory MSD?
The discriminatory MSD (typically 25mm) is the size at which an embryo should definitely be visible. If no embryo is seen at this size, it may indicate a non-viable pregnancy, though follow-up is always recommended to confirm.
Is MSD or CRL more accurate for dating?
CRL is more accurate when available (after 7 weeks). MSD has a larger margin of error (ยฑ5-7 days) compared to CRL (ยฑ3-5 days). Once an embryo is visible and measurable, CRL should be used for dating.
How fast should the gestational sac grow?
The gestational sac typically grows about 1mm per day in early pregnancy. If growth is significantly slower on follow-up scans, it may indicate a problem with the pregnancy.
