What is the Calvert Formula?
The Calvert formula is the standard method for calculating carboplatin chemotherapy doses. Unlike other chemotherapy agents dosed by body surface area, carboplatin is dosed based on target AUC (Area Under the Curve) and renal function (GFR), as carboplatin is primarily eliminated by the kidneys.
AUC-Based Dosing
Target AUC determines drug exposure, allowing individualized dosing based on desired therapeutic effect.
Renal-Adjusted
Dose is adjusted based on GFR since carboplatin elimination depends on kidney function.
Toxicity Assessment
Evaluate nephrotoxicity, hematologic, and ototoxicity risks for safe treatment planning.
How to Calculate Carboplatin Dose
Calvert Formula Steps
Step 1: Determine GFR
Use measured GFR or calculate using Cockcroft-Gault equation
Step 2: Select Target AUC
Typically AUC 5-6 for most solid tumors; AUC 7 for testicular cancer
Step 3: Apply Calvert Formula
Step 4: Apply GFR Cap
FDA recommends capping GFR at 125 mL/min to prevent overdosing
